Please refer to the pre-interview checklist to identify the documents that you must upload to your online application and/or bring with you to the interview. To help avoid delays or a refusal, make sure that you carefully read the interview guideline for your immigrant visa category and follow the instructions.
The National Visa Center attempts to collect documents for all files assigned to Ankara. Some files take an extended amount of time, and some documents can expire before your visa interview. In that case, you must bring (and for electronic PIVOT cases upload) updated documents to your interview.
It is important that you bring all required original documents to your interview. We have created a checklist that will tell you what to bring. Please print the checklist below and bring it to your interview along with the listed documents.
PRE-INTERVIEW CHECKLIST FOR EACH VISA CATEGORY
Diversity Immigrant Visa (DV)
Please schedule and attend a medical examination prior to your visa interview; enroll in the required courier service; and gather the required documents. For more detailed explanation of qualification criteria and required documents please refer to “Instructions for Diversity Visas” at https://tr.usembassy.gov/visas/immigrant-visas/visa-packages-forms/
We encourage you to prepare the below list of documentation separately for each family member in the same order as below before you come to the embassy to avoid any delays in visa processing.
Prior to your visa interview, we recommend that you review the data on your initial E-DV entry. On your initial E-DV application, you must have correctly entered your marital status. If you are legally married you must have listed your spouse, even if you are currently separated from him/her (unless your spouse is a U.S. citizen or Lawful Permanent Resident). Additionally, you must have listed ALL of your living children who are unmarried and under 21 years old. This includes your natural children, your spouse’s children, or children you have formally adopted in accordance with the laws of your country.
Primary applicants must have either a high school decree or two years of work experience within the past five years in an occupation that requires at least two years of training or experience to perform/an occupation in Job Zone 4 has an SVP range of 7.0 to < 8.0 (7.0 to less than 8.0). If you do not meet one of these criteria or found ineligible for an immigrant visa on another ground, your application will be denied, and your application fee(s) will not be refunded.
COMMON DISQUALIFIERS INCLUDE:
- Providing inaccurate biographic information (name, date of birth, place of birth, passport number) at the time of entering the DV Lottery,
- Failure to list spouse and all children at the time of entering the DV Lottery. You must have listed ALL of your living children who are unmarried and under 21 years old. This includes your natural children, your spouse’s children, or children you have formally adopted in accordance with the laws of your country,
- Failure to choose a “Foreign State of Chargeability” to which you are not eligible at the time of entering the DV Lottery – For example, you were born in Iran, and you chose Turkey as your country of eligibility,
- Failure to overcome public charge (e.g., you must provide evidence that you will not require U.S. government assistance during your transition to the United States).

Required Documents
- Confirmation: Appointment information printed from the “Entrant Status Check” on the E-DV website.
- Documents showing that you have either a qualifying high school education OR have two years of qualifying work experience in the last five years immediately prior to application (for the principal applicant only; more information is available on Diversity Visa Process webpage).
- Diversity Visa Application Fee Diversity Visa application fee: Please bring exact change in the amount of $330 U.S. dollars per applicant. You can pay in cash in TL or U.S. dollars, or by credit card. This fee is non-refundable, whether a visa is issued or not.
- DS-260 Form: Confirmation page from the Form DS-260 Application for an Immigrant Visa you submitted online at state.gov/iv.
- Passport: Must be valid for six months beyond your intended date of entry into the United States.
- PTT Registration: Each passport holder applying for a visa must register their passport pickup location with Turkish Post (PTT) before coming to the Embassy online at: https://ais.usvisa-info.com/en-tr/iv or calling +90-850 390 2884 (or 703-520-2490 from inside the United States).
- Medical exam: All immigrant visa applicants are required to undergo a medical examination by a panel physician designated by the consular section. The validity of the medical exam will determine the visa validity, which will be the last day to enter the United States.
- Color photograph: 2 x 2 inches (51 x 51 mm) in size, printed on photo-quality paper, two per applicant.
- Birth Certificate: The applicant must obtain an original or certified copy of their birth certificate and those of any family member immigrating with the applicant.
- Marriage Certificate: If you are married, you must present an original marriage certificate. A certificate is required for all marriages, including those that have been terminated.
- Divorce Decree or Death Certificate: If the applicant was previously married, they must obtain evidence of the termination of every prior marriage. The evidence must be an original or certified copy of a final legal divorce decree, death certificate, or annulment papers.
- Military Card: Required for all male applicants above the age of 17. If the applicant has ever served in the military of any country, they must obtain an original or certified copy of their military record. If the applicant is exempted, they must obtain an original or certified copy of their exemption.
- Court Record(s): If any applicant has been arrested, charged, convicted, or imprisoned in any country, you must obtain a certified copy of each court record and any prison record, although you may have subsequently benefited from an amnesty. Court records should include complete information regarding the circumstance surrounding the crime of which you were convicted and the disposition of the case, including sentence or other penalty or fine imposed.
In addition, copies of court records regarding name changes, age amendments, adoption and similar subjects must also be submitted. Originals, copies and English translations of these documents should be submitted at the time of interview.
- Police Certificate: If any applicant has lived in any other foreign country for more than 12 months, where a police certificate is available or was arrested for any reason, regardless of how long they lived there.
- Proof of finances (assets, savings, job offer in the US. or US. citizen sponsor).
Sponsor Documents: I-134 Affidavit of Support – In order to demonstrate qualification for entry at the time of visa adjudication, diversity visa applicants must demonstrate to a consular officer that they possess the financial resources to pay for reasonably foreseeable living and medical costs. Visa applicants must establish that they would not become a public charge after entering the United States. That determination is based on a consular officer’s assessment of the totality of the circumstances, including the age; health; family status; assets, resources, and financial status; and education and skills.

Spouse of a U.S. Citizen (IR1, CR1)
- DS-260 Form: Confirmation page from the Form DS-260 Application for an Immigrant Visa you submitted online at state.gov/iv
- Passport: Must be valid for six months beyond your intended date of entry into the United States.

- Military Card: Required for all male applicants above the age of 17. If the applicant has ever served in the military of any country, they must obtain an original or certified copy of their military record. If the applicant is exempted, they must obtain an original or certified copy of their exemption.
- Court Record(s): If any applicant has been arrested, charged, convicted, or imprisoned in any country, you must obtain a certified copy of each court record and any prison record, although you may have subsequently benefited from an amnesty. - IRS Transcript and/or 1040 Tax Return Form and W2 Form for the most recent year from your petitioner and any co-sponsor, if any. If you have a co-sponsor, please include their US passport copy green card copy.

Children of U.S. Citizen (IR2, CR2)
- DS-260 Form: Confirmation page from the Form DS-260 Application for an Immigrant Visa you submitted online at state.gov/iv
- Passport: Must be valid for six months beyond your intended date of entry into the United States.

If the petitioner or applicant’s natural parent was previously married, the applicant must obtain evidence of the termination of every prior marriage. The evidence must be an original or certified copy of a final legal divorce decree, death certificate, or annulment papers.
- Military Card: Required for all male applicants above the age of 17. If the applicant has ever served in the military of any country, they must obtain an original or certified copy of their military record. If the applicant is exempted, they must obtain an original or certified copy of their exemption.
- Court Record(s): If any applicant has been arrested, charged, convicted, or imprisoned in any country, you must obtain a certified copy of each court record and any prison record, although you may have subsequently benefited from an amnesty. Court records should include complete information regarding the circumstance surrounding the crime of which you were convicted and the disposition of the case, including sentence or other penalty or fine imposed. If the court document(s) is not in English, applicants should provide English translation.

- I-864W, Request for Exemption for Intending Immigrant’s Affidavit of Support
- OR I-864, Affidavit of Support: From your petitioner and any co-sponsor, if any.
- IRS Transcript and/or 1040 Tax Return Form and W2 Form for the most recent year from your petitioner and any co-sponsor, if any. If you have a co-sponsor, please include their US passport copy green card copy.
- Adoption Documents: If the applicant is an adopted child and the application to immigrate is based on a parent-child relationship, they must submit:
- An original or certified copy of the adoption decree.
- The legal custody decree if custody occurred before adoption.
- A statement showing dates and places where the applicant resided with the adoptive parent(s); and
- If the applicant was adopted when ages 16 or 17, evidence that they were adopted with, or after, the adoption of a natural sibling under the age of 16 by the same adoptive parents.

Parents of a U.S. Citizen (IR5)

- Court documents (original and English translation) indicating the final adoption of the child:
- Documentation about the adopted children’s backgrounds and primary evidence of abandonment: If the children were adopted through an orphanage, you will need to provide to our office an official report from the Welfare Organization. The report must give detailed information about the children’s background and parents’ whereabouts if known. If this is a private adoption, you will be requested to provide similar documentation about the children’s background. This documentation should be obtained from the existing parent of the children. The orphan’s existing parent must give his/her consent for the adoption of the child and must give a notarized statement. This document must state that he/she is unconditionally and willfully forsaken all parental rights, obligations and claims to the child, as well as all control over and possession of the child. In addition to the existing parent’s statement, you should also provide a document from the prosecuting attorney agreeing for the adoption and immigration of the child to US.
- Child’s medical background documents, if any. During the immigrant visa process you will be asked to have your adopted child done the special medical examination by the Embassy authorized medical doctors. - Original “Homestudy” done by the authorized agency.
- Information or documentation confirming that a Primary Service Provider (accredited adoption agency) has been involved in your case. - FormI-864 (Affidavit of Support) and the last year’s income tax return forms (1040 and W-2 forms);

Fiancé(e) to marry U.S. Citizen & live in U.S. (K-1/2)
- Evidence of fee payment: Nonimmigrant visa application processing fee of $265.
- Completed Form DS-160, Online Nonimmigrant Visa Application. You (and any eligible children applying for K-2 visas) must: (1) complete Form DS-160 and (2) print the DS-160 confirmation page to bring to your interview.
- Passport: Must be valid for six months beyond your intended date of entry into the United States.

- Letter of Intend to Marry: The I-129F petition is valid for four months from the date of approval by USCIS. A consular officer can extend the validity of the petition if it expires before visa processing is completed. If your petition expires before the visa interview takes place, you are required to bring to your interview a signed letter from your petitioner confirming that you both remain legally free to marry and that you will marry within 90 days of your arrival in the United States.

However, they must show evidence that they are not likely to become public charges while in the United States. Applicants may submit Form I-134, Affidavit of Support, or may submit alternative forms of evidence to overcome the public charge provisions of the law, INA 212(a)(4).

Police Certificate with Archived Records – Arşiv Kayıtlı Adli Sicil Kaydı: They are available from the court and also can be obtained from e-Government. When requesting the archived police certificate (arşivli kayıtlı adli sicil kaydi), applicants must specifically request that both current and archived records be shown on the document.
Iran
Birth Certificate – Shenasnameh: Renewals of old birth certificates or replacements of lost birth certificates are available by applying in person at the registry office.
Police Certificate: Not required
Iraq
Birth Certificate – The Family Book “Qayd 57/Copy of Entry 1957: Issued by the General Directorate Of Nationality (Mudeeriyat Al-Jenssiyah Al-Aama). In addition, Iraqi Electronic Personal Data Registration may be provided in lieu of 1957 Individual Data Registration.
Police Certificate – Shahadat adam mahkoomiyah: Applicants must appear in-person to apply for a police certificate to The Directorate of Criminal Information “Modeeriat Aladellah Aljinaeyah” in Baghdad.
Please note Police certificates issued in the Kurdistan region (Erbil, Sulaimaniyah and Dohuk) are not acceptable for Immigrant Visa application purposes. The police certificates issued by the Directorate of Criminal Information are frequently replaced and updated.
Syria
Birth Certificate – Extract of the Individual Entry in the Civil Register and/or Family Extract
Police Certificate – Non convicted certificate: Available to Syrian citizens residing in Syria but may be unavailable to citizens living outside the country.
Afghanistan
Birth Certificate – Tazkira
Police Certificate: Not required
